It's a play on words, or rather a play on meanings. When you have a cold/flu, your nose is said to 'run' -- ie, stuff comes out of it. Another variation on this joke is:

Patient: "Doctor, doctor, my nose is running."
Doctor: "Well, run after it, then!"

There are hundreds of these so-called 'Doctor, doctor' jokes around. The joke is usually in the doctor's reply and many involve plays on words. Here are some more:

"Doctor, doctor, I've swallowed lots of coins."
"Take this laxative and we'll see if there's any change in the morning."

"Doctor, doctor, I keep getting shorter and shorter."
"Just wait there and be a little patient."

"Doctor, doctor, I've got the wind [=flatulence]. Can you give me anything for it?"
"Try this kite."

"Doctor, doctor, people keep ignoring me."
"Next!"

And so on.
