Subject: near-miss error
In hospitals – where prescribing errors are often caused by unintelligible hand-written prescriptions – patients experience an average error rate of 2% in the proper prescription or dosage of drugs, and a near-miss error rate of 11%!

Как бы вы перевели это выражение "near-miss error" в данном контексте? Ошибки по недосмотру - мне не нравится. Это не совсем то, что надо.

near-miss error - ошибки, которые успели заметить и исправить. Пациент не успел пострадать
