Please help define and translate this abbreviation:

I need to know what the letters stand for in Russian and what the Russian words mean in English.

The phrase is used in the following manner:

It is used in the header for what appears to be a fax (facsimile) transmission. It comes after "From: Company name." It is followed by a number and then the date, time, and page number. This is a financial document giving the bank account information for two companies (a buyer and a seller).
