Subject: Определение сердечной недостаточности. Помогите понять смысл изречения. Лекция (со слайдами) по ведению пациентов с сердечной недостаточностью (СН). Дается новое современное определение СН.I mentioned the new universal definition this was published jointly in the Journal of Cardiac Failure and the European Journal of Heart Failure. And it was the work that took us some while multiple experts across the world and we've really simplified the definition. So, you see on the left is the critical components and the attempt was to make it more democratic, so adopter without access to specialists’ investigation, or someone coding, looking at a hospital episode with a hospital discharge summary, could with confidence, say this looks like heart failure. Интересует крайнее, третье предложение. Смысл не совсем понятен. Мой вариант такой: Слева Вы видите критические компоненты. Определение более демократичное, чтобы медработник без доступа к исследованиям специалистов, или присвоенному кем-либо коду заболевания, просто на базе больничного эпизода или выписного эпикриза мог с убежденностью сказать, что это похоже на сердечную недостаточность. Благодарю заранее. |

My try: Итак, слева вы видите критические компоненты, и (нашу) попытку сделать всё более упрощенным, чтобы сотрудник без доступа к истории конкретной болезни или не знакомый с системой кодирования (международная статистическая классификация болезней), смог бы взглянуть на выписку и с уверенностью сказать, что имеет дело со случаем СН. |

In the 3rd sentence that includes: * 'So, you see on the left is ........' => 'So, what you see on the left are ........' * 'so adopter' -- there's an article (an/the) missing before 'adopter'. Also, I don't know what 'adopter' means here. * 'someone coding' -- without further context I don't know what this means either. |

For more context: So, you see on the left is the critical components and the attempt was to make it more democratic, so adopter without access to specialists’ investigation, or someone coding, looking at a hospital episode with a hospital discharge summary, could with confidence, say this looks like heart failure. So, the critical components were symptoms and/or signs of heart failure, typical of heart failure. And the next word is critical, caused by a structural and/or functional abnormality. It can't just be in the presence of. The doctor make the assessment that the classical symptoms were caused by the observed cardiac abnormality. Then in addition to that, they require some independent collaboration. And that could be by objective evidence of congestion, pulmonary or systemic, and that might require an imaging status such as a chest X ray, could be documentation of raised JVP, could be hemodynamic assessment, or a measurement showing elevated natural peptide levels, as they're the two confirmatory alternatives, for the definition. ochernen: Thanks for that. The English overall is about 8.5-9/10. Unfortunately I still don't know what 'coding' means here, although it may have a specific medical meaning here that isn't in any of the medical dictionaries I've checked. Has 'coding' been mentioned earlier in the text? I'm not a cardiac/medical expert, though, so won't guess at the meaning. Ideally you need to ask someone with a medical background. 'It can't just be in the presence of.' is imperfect English. Could it perhaps mean 'It can't just coincide with [it]'??? That's just a wild guess by me, though. Again, someone with a medical background may be able to make more sense of it. |
