There is a tendency toward a silo management approach
The very top senior management are unaware of the specific issues impacting down below
Down below there is suspicion, arrogance and confrontation driven by big picture understanding and the threat of interference.
Significant Drilling cost and dry holes complicates project (financial) Management with senior management
Therefore, policies and procedures established typically by the finance department (based on classic CPA principles)
These policies are typically not optimized for something as complex as a drilling operation.
The drillers, are focused on well design with no **control on G&G
