да это же согласие! Кста, учтите, что в Европе нет такого док. как согласие, поэтому стоит задуматься о переводе "Согласие" как "Parental Consent" По сабжу: есть несколько шаблонов согласий, в зависимости от нотариуса. Навсидку: In accordance with Article 22 of the Federal Act of the Russian Federation on the Procedure of Exit Out of the Russian Federation and Entry into the Russian Federation we, XXX and XXX, hereby give our consent that Ms. XXX shall assume responsibility for the life and health of our underage daughter XXX and shall take all and any necessary decisions related to the protection of our child’s rights and legal interests, including those related to medical intervention if necessary as well as file any applications and receive any documents including visa. |

| Зачем спорить-то? Ваш вариант тоже правилен, только Exit from чаще встречается, а не Exit Out of. |
