как я понимаю, это может быть, соответственно

рекламная страница-проспект (форма заказа книг/журналов издательства, вшитая в страничный блок)
рекламный проспект-вкладыш (тоже самое, только в виде отдельного информационного листа/формы, часто такой можно встретить в журналах)
