Subject: FORM OF SURRENDER AND REQUEST FOR TRANSFER gen.

Здравствуйте, помогите пожалуйста перевести заголовок документа и следующее предложение:

"FORM OF SURRENDER AND REQUEST FOR TRANSFER

I/WE, the within written Member(s) .....................
of...................."

буду очень благодарна. Для контекста ниже приведу следующий абзац:

of the written Member(s) hereby surrender this Membership Certificate pursuant to Clause 15 of the Constitution of the Company subject to the issue of the new Membership Certificate in accordance with the Request contained below and hereby agree to use my/our best endeavours at the cost of the person or persons named below to procure that such a person or persons is/are admitted to membership of the Company in accordance with the Constitution and pending such admission declare that I/we hold my/our interest as a Member(s) of the Club (to extent that such interest may be so held) upon trust for such person or persons and I/we

Спасибо, но как может быть "форма отказа" если далее пишется "to the issue of the new Membership Certificate" то есть выпуск нового сертификата?
Можно ли "surrender" перевести как "выдача"?

и "Запрос на передачу" чего?
О "Property" речи не идет в тексте.

отказ от старого в случае выдачи нового.

форма "Вашего" заявления (кстати, типовая) не сама по себе существует, она является приложением к уставу некоего клуба, и в тексте устава transfer касается не сертификата а некоего объекта недвижимости "Property"

я просто никак не могу въехать)))
"the within written Member(s)" это значит "Участник(и) описанные в документе (то есть в некоем уставе)"?

и "to use my/our best endeavours at the cost of the person or persons named below" это "прилагать все усилия за счет человека или людей представленных ниже"?

Участник(и), указанные/поименованные/т.п. далее/ниже/т.п.....

"..to use my/our best endeavours at the cost of the person or persons named below to procure that such a person or persons is/are admitted to membership of the Company..."
смысл в том, что некто будет прилагать все усилия, чтобы такое лицо(а) приняли в члены (...), при этом это лицо(а) оплачивает все связанные с этим расходы
